Deeply appreciated Charles Subject: Concerns Regarding Communication and Conflict in reality of Documentation of my Home Bound Status I am writing to express my concerns regarding the ongoing issues related to my home health services, specifically the discrepancies between the information provided to me by Act Home Health and that communicated by the various in-home care providers. Following my recent review on Google, I was contacted by a representative from your agency to discuss my concerns about the services ordered by my physician and approved by my insurance provider. During this conversation, it became clear that there is conflicting information between what I am being told by the in home providers is being documented by the in-home care providers and what I have been told by Act Home Health representatives. Moreover, I have been informed that the need for additional visits is primarily to what is being documented due to the lack of updated communication from the Home Health Agency regarding orders approved by my physician. This lack of coordination has resulted in confusion and has impacted the quality of care I am receiving. One significant issue is that patients do not have visibility into the documentation regarding their home care. It appears that the in-home providers are not sharing this information with patients, which raises concerns about the integrity and transparency of communication about care being provided. It seems there is a mutual attribution of responsibility between Act Home Health and the in-home providers regarding the failure to keep all parties informed. I would not recommend Act Home Health to anyone regarding in home care management. Subject: Concerns Regarding Patient Care Dialogue To: Courtneyc/o Act Home Health Agency I am writing to express my concern regarding the agency's approach to open dialogue concerning patient-related care issues. It appears that the primary focus of the agency is to deny and defend its shortcomings instead of engaging constructively with patients to address their concerns. This defensive posture not only undermines the importance of patient feedback but also fosters an environment of conflict rather than collaboration. Patients should feel heard and valued, yet the current approach seems to prioritize the agency's interests over effective patient care management. A shift towards a more open and transparent communication style is crucial for rebuilding trust and improving patient relations. It is essential for the agency to prioritize resolution and understanding to enhance the overall experience for patients, rather than positioning itself in opposition to those it serves. I would wholeheartedly recommend this company. Our nurse Renise took very good care of my family member. She was very professional, knowledgeable and passionate. She was very personal with us and made us feel very comfortable. I can’t thank her enough for the care she provided our family , when we needed it the most.

One thing I love about her is how she interact with her patients, communicate with family also she has a beautiful smile always smiling it make the patient feels good and comfortable. I called today inquiring about continuing PT for my 99y/o uncle who is a WW2 veteran and as I tried speaking with Brandy who at first refused to give me her name, but she became increasingly loud, argumentative and unprofessional about me inquiring about the status of the PT orders from the VA. I tried explaining to her that I had requested orders before this week’s blizzard and that they should have received the orders last week but the more I tried explaining the louder she became and her lack of consideration for me as concerned family member, as I am his niece, but I am also a nurse that clearly understands the process and need to have orders to continue care. She then insisted that my uncle has not received wound care from Acts when in fact Derrick, the nurse assigned to my uncle serviced his wounds today so that in fact proved to me that this agency clearly doesn’t have a handle on the care that is being rendered which makes me very concerned about this company’s ability to coordinate care for my uncle or any patient’s care for that matter. Derrick has been wonderful but I don’t recommend using Acts Home Health because of this lack of professional communication and lack of awareness of the care being given😧
