I was referred to Centerwell Home Health after a stay at Rosewood rehab center after discharge from the hospital. I was evaluated for OT, PT and nursing. I just finished the last of my PT therapy and continue on skilled nursing for control of my CHF. The staff are great they work together like one big family. They always get back to me when I call to ask something and tell me about programs that are for seniors. If you want a place that will treat your loved ones like they would their own family CenterWell Home Health is the place for you.

Tracy is my nurse I love having her come out, she explains my meds and what they are for. I feel like she really listens to me. Tracy keeps a close watch on my B/P and my heart rate April is my physical therapy's I enjoy the days she comes out. She explained what l need to do to get my strength back. She also keeps a close watch on my B/P and my heart rate. I would recommend both Tracy and April they both treat me like a person not a number. They are both very kind, friendly, understanding and knowledgeable in what they do.

Great a year ago. Horrible now. Terrible communication. Would not recommend. 24/7 Nurse line was even worse. Did not take into consideration patients limitations or pain and advice did not agree with primary care doctor. Discharged without coming. They call the night before and do not understand that patient has doctor appointments and family members work. Occupational Therapist (Angie) is fantastic - but had a lot of issues with nursing care.

Nicole Buckner and Rich Reid have both provided first class and caring support and help for me during my recover. Having worked with both of them before (during recovery by wife) I expected nothing less. Both, but particularly Nicole go that extra mile to provide caring and effective support. CareGivers present during her visits have also commented how she always asks more questions and follows up and very obviously cares so much more than other of your nurses. Raises and promotions well earned and should be awarded if possible!

PT Laura was wonderful. She was encouraging, friendly and focused on my mom’s needs. My mom has had noticeable improvement and increased confidence in her functional abilities. OT Cherlyn was not as impressive. There was very little hands on therapy unless specifically requested. Nicole, the home health nurse, was kind, attentive and responsive.
