My experience with fourseason rehab was not at all good I put my trust in them based of the promise they put out to the public and how great they say they are but they loose my trust 100% I will never trust them again my family member wasn't treated with the care they promised,it's a very sad thing to see how people treat the elderly and for you to do this job u have to love it to be good at it I feel like there should be stricker rules in place when taking care of the elderly it hurts my heart to see them being mistreated or egnored. fourseason I'm sorry but u did not win my star it's a 0% for me And also the price does not fit the service

Clean, comfortable, and absolutely professional!!! 100% recommend this facility.Thank you to all the staff here for taking such wonderful care of my mother... All the doctors, nurses, and nursing assistants ,physical therapy , All are outstanding. I can't express my gratitude enough... They are all amazing.I can honestly say that my mother is very happy and at peace because she received such excellent care from everyone at Four Seasons Center. Once again, i want to thank you a very important person, Ms. Belen and her team . also thank you Four Seasons Center for helping our family ❤️❤️

If you have your loved one at this facility prepare to advocate 1000%. They leave your family member messy for hours blatant patient neglect! The process to get your loved one seen outside of the facility is a long process and let’s not talk about how they don’t answer the phones! Only time you can speak with healthcare personnel’s is if they call you or you go in person. In person you literally have to complain and reception has to call constantly and the wait is forever for anyone to come to talk to you. They rather take messages and communicate with you through the reception desk, very rude!! The doctor Samir Farhat, is rude unprofessional argues with family member in presence of patient loudly, rolling eyes and rude at bed side! Arguing about patient medical decisions in the presence of the non verbal critically ill patient, causing additional distress to the patient, having to hear of the burden they are to the facility, is disheartening. He does not pay attention to detail and neglects patient unless it’s convenient to him and his staff, in an effort to make their jobs lighter but zero care for the patient. They document lies, reading the notes you would think they are doing everything as expected but in reality, it’s the advocate who has to get things in motion. There were several instances I had to initiate a hospital visit because I found my love one in utter distress while he and other staff sat by nonchalantly. Let’s not talk about the hippa violation that as been breached, with patient information. It’s almost as if, they are running a facility without an effective healthcare team, only want easy patients to care for. The doctor is 90% of the times clueless about his patients health woes and often shy away from answering pertinent questions. Everything is a top secret, they are programmed to avoid questions our fear exposing each other. The best department there is physical therapy! They actually put in work but the other areas are lacking and SOCIAL WORK DOES NOT ADVOCATE OR DO THEIR DUE DILIGENCE for your loved one, waste of time trying to resolve anything through them. Oh and ensure they don’t change your loved one’s insurance so they can benefit from insurance money while the care is lacking! Run don’t walk going there. There CMS rating needs to reevaluated and the states needs to look deeper into this rehab center. They will treat your loved poorly even with advocating and gets annoyed when you ask questions about their care. DO NOT WALK, RUN!!!!!

I want to thank the staff of Four Seasons Nursing Home especially the 4th floor staff for their excellent compassionate care they provided for my mom, Joyce Clarke. She was well cared for by her morning CNA, Ms. Boodie who is exceptional. She went above and beyond her duties to take care of her patients. Very kind and compassionate. Her other CNAs, Mrs. Samuel, Ms. Dona, and Mrs. Franklin, worked with her mostly as well. They were all amazing, especially the CNA, Mrs. Phillipe, from the 2nd Floor. They all treated my mom like she was their mom. My mom was well cared for on the 4th floor whenever I went to visit. The staff were friendly. The nurse supervisor, Ms. Bernard, always had an encouraging word to give to me. Nurse Ingrid, nurse Cassandra, nurse Kingston, and nurse Emanuel were phenomenal. I would ask them questions and I would always get an answer. The night my mom passed away, Mrs. Samuel and nurse Emanuel were there making her comfortable until her last breath. The Nurse Practitioners Ms. Kerry and Ms. Mel both were great in her care. Thank you for the wonderful care, compassion, empathy, and kindness. I know caring for my mom was challenging at times but you all never gave up on her. The 4th floor staff has become my second family. I will never forget any of you. To the best social worker Heather, Mrs. Matilde, and Henry thank you. Such an amazing staff. Words cannot express my gratitude thank you all again from the bottom of my heart. -Marva

The aides and nurses are really nice and attentive to my dad. He is on the 6th floor and they look out for him and make sure he's comfortable. Thank you to Ms Coley who goes in there talks to him and make sure he's nice and clean.
