Thank you to the staff at Transitions for supporting my family during such a difficult time. Elisa, the social worker, went above and beyond to communicate, show her care and keep us informed. I’ve never met a social worker as attentive, dedicated and caring as her. Amanda and David, the two nurses who cared for my dad, did an excellent job tending to his needs while showing professionalism, patience and compassion. I am forever grateful for your staff, the volunteers and the work that Transitions is doing to allow people to experience care, dignity and respect before their homegoing.

My mom passed away in early July of this year, so it’s taken me a few months to feeling up to doing this review. But I knew I wanted to express my incredible gratefulness for Transitions’ help in her final weeks. My mom, 85, was living independently when she had a fall in the shower. This led to 3 weeks in the hospital, then 2 weeks in rehab, where she contracted pneumonia. Back to the hospital for 2 weeks, where she declined, and ultimately ended up in a high-skilled nursing facility for 2 weeks, where she ultimately passed away. Transitions became involved during her stay at the nursing facility. I was at a maximum stress level (I am an only child). Transitions arranged for ALL of her equipment and supply needs (within only 24 hours), including a rented hospital bed, oxygen supplies, incontinence products, etc. They also worked with the facility staff on pain medication administration. A Transitions nurse checked in on mom nearly every day, in-person, and promptly called me with an update. They had a faith-based person visit her, and had volunteers stop by to keep her company (which gave me incredible relief from feeling like I had to be there 24/7). When mom passed, they quickly arranged for removal of all of the equipment and supplies - again, relieving me of that worry. Transitions provided exceptional care and support to my mom. But just as importantly, they relieved my burdens at a time when I was emotionally raw and physically exhausted. I cannot thank them, or recommend them, enough.

My father was under Transitions LifeCare for Hospice in skilled nursing facility for approximately 6 months until his passing August 21, 2025. He received stellar care from all staff that he and our family encountered. We were provided with precise details on how and when services would be provided. Transitions provided prompt 24hrs care and support to my Dad and family to include adjusting his care plan efficiently as his needs changed. Every visit completed by staff came with concise updates to our family. We had the best RN Case Manager, Sarah A Upchurch RN,CCM who was professional, provided timely communication with family and nursing facility, approachable, sensitive, warm and a great advocate for my Dad and our family. Transitions LifeCare provided an extra layer of care that was needed to ensure my father received the care he deserved before and after he transitioned. Transitions continued to f/u with our family for Grief Care if needed. The entire experience with this agency and staff was exceptional.
