Denied medical care services, employee(s) violated the HIPAA laws, and the agency of Warren General Hospital, yet again, covered up their negligence to prevent accountability. This agency was not chosen for services in May 2025, but a fax was sent to the Home Health services of WGH because of insurance reasons. On June 2, 2025, Carrie Burlingame of the WGH home health services called me to state I was not qualified to receive their services. Well, that's okay, because I really did not request your services; it's just that another agency faxed you medical orders because of insurance reasons. Carrie Burlingame proceeded to tell me she went through my medical records at WGH, dating back to January 2019. A red flag for sure, as she had no reason to "snoop" into six year old medical records. But in those records at WGH- it would disclose the exact negligence that I incurred at WGH by Paul Shaderowfsky, a radiologist that informed in back in 2019 that he not only knew how to perform the medical procedure of a myelogram, but that he actually performed them. Truth be told by the medical board, Paul Shaderowsfsky does not perform such a procedure at WGH, in fact, no one does. The so-called doctor of radiology caused an Iatrogenic Cerebral Spinal Fluid Leak when he punctured my dura, and then caused Chemical Meningitis when he inserted contrast medication into my dura. For the next six years, I fought to live, and continue to do so today with the effects of his negligence and his ego to perform a procedure he claimed he knew how to perform. So, why was Carrie Burlingame in my medical records from six years ago, and why was I suddenly being called on a Monday morning to be told WGH home health services would not follow the medical orders for home health physical therapy? In order to be denied medical services, the agency had to have followed the law, and contacted the insurance company to request approval of services. That was never done, the agency took it upon themselves to deny medical care. Carrie Burlingame claimed "There's no medical diagnosis" on the order. Sure there is, I have a copy right in front of me Ms. Burlingame. Her response was "I went back into your medical records to find a diagnosis." Knowing the medical field as I do, I knew that was a lie. When Ms. Burlingame was told she violated the HIPAA laws by snooping in my medical records, she responded, "You're probably right." The nightmare of this hospital and the negligence they cause is unfathomable. Did they ever call the insurance company to get authorization? No, they did not. This group of people took it upon themselves to deny medical care and then collaborated with the hospital staff itself to make sure they would not be held accountable for negligence. I pray I can help at least one person from being harmed by this hospital and it's unethical staff. It's not legal to "snoop" into a person's medical records, it's not legal to deny someone home health services without even contacting the insurance company, it's not ethical to take it upon yourselves to deny medical treatment; but then maybe the Grace of God was watching over me~ as Paul Shaderowfsky had already harmed a persons life and forever changed it by his negligence and deceitfulness. The Home Health agency is mandated to provide a patient, in writing, a reason for denial of medical services; this agency has taken steps to deny even this basic obligation. One big coverup of negligence leads to more coverup of negligence within the facility of Warren General Hospital and it's home health service.
