The communication process with HomeChoice is not a help to patients and families as you never know when to expect a visit. You will get a call with very short notice making it difficult and chaotic for the family. The schedule changes without notice and the care received is inconsistent; therefore, I called often in an effort to coordinate care with the family schedule. The nurse that came out to assess did not provide any nursing services or even make note of a significant wound even after I’d given her a full history and detailed information about my mother’s condition. When calling HomeChoice to inquire about wound care, I was instructed to clean wound with dial soap. This is very serious and a profound neglect on the part of the agency. I specifically requested wound care and supplies as the agency did not make note of it. I was told that the nurse was new and a travel nurse that the administrator had not yet met. The same nurse was sent back out and still did not instruct the primary care giver , me, on how to clean and dress the wound. Patients can die from untreated or improperly treated wounds. HomeChoice continued to fall short in regards to getting lab work drawn, and providing adequate aid assistance. Due to the delay with the lab work, we missed an appointment with her primary care doctor. The care that was as expected came from the Occupational and Physical Therapists Wendy and Lisa, along with the second nurse Levetia. My mother had to be hospitalized, she hadn’t been properly assessed by the nurses who had come out. Next, she was discharged from home health service without notice. Again, communication with this agency is problematic. I did not find out about the discharge until calling regarding the schedule after no visit or call during the week. The administrator, Sandra, claims that they are not understaffed; however, she advised that her nurses refused to come back out and see my mother, citing that they felt there was nothing they could do to please me. It’s not me they need to please, they only need to provide appropriate care. The refusal to provide care puts my mother’s health at risk. When asked to speak with a social worker, the administrator advised that they didn’t have one but was interviewing. The two stars are earned by Wendy and Lisa; otherwise, it would have been zero stars if that were possible based care and communication. All of the staff were pleasant to talk with when they came out; however, not all provided appropriate care, intentional care, or showed diligence in the care given. It was get in and out as quickly as possible. HomeChoice does not inspire confidence in home health care services. Patients are not their priority. So, if you find yourself in a similar situation. Don’t waste your time or energy with this agency; call your insurer and find someone else who can help you and your loved one with compassion, empathy, dignity, and professionalism. Being a caregiver is hard and your energy should be focused on helping your loved one heal, not struggling with agencies that just want your money. Update 12/19 - After following up with hospital case management and primary care physician, I found that the administrator provided false information about my mother’s case. So I have to take away the stars. If I could give a zero for dishonesty, I would. The lowest I can rate is one star.

Joseph Ardiel Lubrico is an excellent Therapist. He is professional, friendly, caring and compassionate. He has a wonderful sense of humor and engages my Mother in conversation which she enjoys very much. He is very knowledgeable and skilled. He knows when and how much to push her in her therapy and also when to slow her down or give her a break if she is trying to do too much. He gave us a recommendation for knee braces that are perfect for her and relatively inexpensive. The Ortho doctor advised they were best for her to stay with for now when asked if she should have different braces. We first worked with Joseph a few years ago when he was with a different agency. When my Mother needed home therapy again I tracked him down because we only wanted to work with him. He speaks well of HomeChoice Health Services and we have had a good experience all around with them and certainly with Joseph. We cannot recommend him/them highly enough.
