I do not recommend this agency, especially for ostomy patients. My mother was enrolled with this agency after she was discharged from the hospital after having colostomy surgery. Everything was so new for us, and we were advised that the NP was going to help us and educate us. The first visit was the Director of Nursing, her name is Janeth. She did not even put the ostomy appliance on correctly, and it failed right away but Janeth assured us that we would be getting more supplies which this agency failed to provide. The NP that was assigned to my mother was Crystal Hernandez. Although Crystal was polite, she was unreliable, inexperienced, and unorganized. We have been with Home Sweet Home for 6 weeks. We are supposed to see the NP once a week. We only saw her 4 times, and it was at random. She would text the same day asking to come by in less than an hour. She also would only bring bits and pieces of ostomy supplies and force my mom to wait until the following week for the supplies she forgot to order, to which she would not even show up at the time she'd say she would. I even drove to the office once because Crystal kept saying she would bring the supplies, but then we wouldn't hear from her anymore until the next week when she's "ready" to come. Also, Crystal has not even attempted to change or even touch my mom's ostomy bag. The past 2 weeks, Crystal didn't bring any supplies, and we have not seen her. I had to purchase supplies out of pocket for my mom. Crystal also knew we had a plan to get my mom a physical therapist so we can work on building her strength before she starts chemo therapy again this month. 4 weeks have gone by just for us to find out that the referral for a PT that i gave to Crystal was never submitted. Our entire experience with this agency was just bad. The only thing they did right was discharge us the same day so we could receive services elsewhere. I am glad that I was able to help purchase the supplies for my mom when she ran out, but this sort of thing shouldn't happen in the first place and I cannot imagine what it must be like for other customers who have Crystal as their nurse or this agency as their Home Health Aid and are fully reliant on them for their needs. Please, if you want the best care for your loved ones, avoiding this agency, Janeth, and Crystal Hernandez is the best choice to make.

In my opinion Janette there could care less if she notifies you of any changes and lies about re-admitting and conceiling the fact that you are being discharged from the agency, also I reported being sick and was unavailable to be seen a day or two which of course I notified them but still is no means to concern them for calling my doctor and telling them I was non compliant. All in all thank you to the nurses that where helpful and , Lastly I hate they are using a sign pad that takes up the whole screen of the cellphone they make you sign without knowing what you are signing.

Aka Physical Therapy Center. I have been here for PT multiple times for different ailments. My father-in-law has used their homebound services and my mother is currently receiving PT at the clinic. They have had absolutely positive visits and have enjoyed every encounter. I have as well. Allan is an awesome therapist. He listens to you, asks you questions and is constantly assessing you. I love his dry needling. I won't go to anyone else. The entire staff is welcoming, friendly and professional. You really feel cared for here...not on an assembly line. They have two locations: east off of Joe Battle and northeast. If I could give 100 stars I would. Much love to Allan and his team. They have taken care of me over the years as well as my family. XO
