Charles, the liaison, was outstanding in guiding us through the process. He took the time to explain every step, answered all of our questions, and ensured that we felt comfortable with the care plan. His attention to detail and commitment to patient care were evident from the start. Our experience with the nursing staff was equally impressive. Jasmine, our nurse, was phenomenal. She was thorough, attentive, and provided top-tier clinical care while also offering warmth and reassurance. Having someone as skilled and compassionate as Jasmine made all the difference. Patty, the admitting nurse and case manager, was another standout. She ensured that the transition into home health services was seamless. She took care of all the necessary details, coordinated with other healthcare professionals, and made sure that we had everything needed for a smooth start. Her knowledge and professionalism were evident from the moment she walked through our door. Haley, the ADON, was fantastic in overseeing our care. She was always available to address any concerns and went above and beyond to ensure that everything was handled efficiently. Her leadership and dedication to patient care were truly commendable. And when it came time for discharge planning, Patty once again stepped up, ensuring that everything was coordinated properly and that we had the necessary follow-up care in place. Her ability to manage the complexities of our case with ease and professionalism made a world of difference. What really stood out about Living Hope Home Health was their commitment to patient-centered care. They didn’t just check the boxes—they truly cared about my family’s well-being. They took the time to listen, adjust plans as needed, and ensure that we always felt supported. I have worked with many healthcare providers in my career, and I can confidently say that Living Hope Home Health is one of the best in the industry. Their team operates with the highest level of integrity, skill, and compassion, and I am beyond grateful for the care they provided. From the initial contact to our subsequent discharge, I’m pleased to report that Living Hope is an outstanding home healthcare organization. Rather than being overwhelmed by the need for multiple home therapies after our family member’s extended hospitalization, Living Hope staff provided supportive attention to detail, compassionate care and timely responses to our questions/concerns. The scheduling and coordination of the physical, occupational and speech therapists was seamless. I cannot offer enough praise for the therapists who delivered thoughtful and sensitive care that focused on sustained improvement. Because of all of this, including the steadfast offers of emotional reassurance and psychological support, I highly recommend Living Hope Home Health. My Dad receives care from Living Hope. They come see him, do blood work if necessary and checks his overall health care needs. I am impressed by the team of nurses and nurse practitioner. Sebie, one of his nurses, is very sweet and really cares for my Dad's well being. While she is off on vacation, Briana has stepped up in her absence. She is sweet, caring and respectful of us. Briana also love's my dogs as if they were hers! They love her back as well 💜❤️💜🐾🐾 While I have been recovering from a recent amputation, they both are understanding and really care for the 4 of us! I am truly thankful 🙏 for Living Hope and the care my Dad receives. I would highly recommend Living Hope to anyone in need of home health-care. They are exceptional in what they do! I have had experienced home health many times, but Living Hope Home Health care has excellent employees. Brianna, my nurse, goes above and beyond to help me and she is such a kind and caring professional. Ernesto, my PT tech is so uplifting and encouraging. Somehow he seems to motivate me to keep pushing through to advance movement and has now got me to wheelchair to exercise!! From Michelle OT to Brenda my shower aide I was so moved by the kind and cheerful support my whole care team has given me. I will always request Living Hope home care. They are like family to me. My team is the best and the heart of home healthcare and represent what home care services should be.
