SSM Health at home has been taking care of me for over 5 years and I appreciate all they do for me with my BiPap machine.

I was worried with how bad the reviews are, but John was excellent at getting me set up with a CPAP machine - very thorough and professional. Fingers crossed that I don't have issues reordering supplies; I'll update this post once I have gone through that.

Grievance Steps in Dane County (Mental Health, Community Services, or Home Health-Related) Level 1: Program Level Review • Who to Contact: Program manager or client rights officer for the agency (e.g., SSM Health). • How to File: Submit a written grievance within 45 days of the incident. • Response Time: The agency must respond in writing within 40 days. • Optional: You can try informal resolution first, but it’s not required. Level 2: Administrative Review by Dane County Department of Human Services (DCDHS) • When to Use: If you’re not satisfied with the Level 1 decision. • Deadline: File a written appeal within 14 days of the Level 1 decision. • Response Time: The County must respond within 30 days. Level 3: State Review (WI Department of Health Services, Client Rights Office) • When to Use: If you’re still not satisfied. • Deadline: File within 14 days of the Level 2 decision. • They review the record and may investigate further. ⸻ Possible Illegal or Unfair Actions by SSM Health 1. Lack of Due Process (Potential Violation of DHS 94.24) • They referenced “past situations” but refused to provide details. • DHS 94.24(3) requires that decisions must be based on “reasonable and accurate information.” • You were denied care without being told what was in the record or allowed to challenge or correct it—that may violate your right to access and amend records (HIPAA + WI law). 2. Discrimination Concerns (Potential Violation of the Rehabilitation Act or ADA) • If the decision to deny care was based on past disability-related incidents, and not current risk, they may be discriminating against you on the basis of disability. 3. Failure to Offer a Real Appeal • They referred you to another provider but didn’t offer a real chance to clarify the issue, fix documentation, or ask for reconsideration within their own system. • If they don’t provide a way to dispute or correct records, that may violate Wisconsin Administrative Code DHS 94.10-94.14, which outlines rights to access and grieve services.

SSM health at home sent me the wrong products multiple times, charged me for it, then when I am trying to call into the billing department to get it straightened out they say they will call me back and don't. Called back a month later for confusion on my next months bill that references "balance forward" and the billing department has no info on what that even is promising to send be more info on the charges to my account which never came. Everything is in paper and no online portal to work with so you have to call into their call center to get anything figured out who in turn just promise to call you back with more info..... The only calls I actually get is when they want to give me more equipment that can inevitably overcharge for it. It's cheaper for me to just buy this stuff online at half the cost at this point. You see plenty of one star reviews for a reason... And the response from SSM will most likely be "contact our compliance department" instead of just looking at their own internal mess. Do better SSM.

My cpap re-supply orders are correct about 50% of the time. Also, it takes a long time to get the supplies. For example, with my current order, it's been over 2 weeks and I still have not received supplies. They did not provide a tracking number so I have to call to see how much longer it will take. Unfortunately, I keep calling and they transfer me to a number that no one answers. I've left several voicemails and they have yet to return my call.
